چکیده

Remote ischaemic preconditioning reduces the risk of myocardial injury within 4 days hip fracture surgery. We aimed to investigate effect remote on incidence major adverse cardiovascular events 1 year after performed a phase-2, multicentre, randomised, observer-blinded, clinical trial between February 2015 and September 2017. studied patients aged ≥ 45 years with minimum one factor. Patients were allocated randomly applied just before surgery or no treatment (control group). was upper arm tourniquet in four cycles 5 min ischaemia reperfusion. Primary outcome occurrence A total 316 group 309 control group. Major occurred 43 (13.6%) compared 51 (16.5%) (adjusted hazard ratio (95%CI) 0.83 (0.55–1.25); p = 0.37). Fewer had infarction (11 (3.5%) vs. 22 (7.1%); 0.48 (CI 0.23–1.00); 0.04). did not reduce The outcomes non-cardiac needs confirmation appropriately powered randomised trials.

Remote ischaemic preconditioning reduces myocardial injury in patients undergoing cardiac surgery with cold-blood cardioplegia: a randomised controlled trial.

BACKGROUND Remote ischaemic preconditioning (RIPC) induced by brief ischaemia and reperfusion of the arm reduces myocardial injury in coronary artery bypass (CABG) surgery patients receiving predominantly cross-clamp fibrillation for myocardial protection. However, cold-blood cardioplegia is the more commonly used method world wide. OBJECTIVE To assess whether RIPC is cardioprotective in CABG...

Does remote ischaemic preconditioning with postconditioning improve clinical outcomes of patients undergoing cardiac surgery? Remote Ischaemic Preconditioning with Postconditioning Outcome Trial.

AIMS The aim of this study was to evaluate whether remote ischaemic preconditioning (RIPC) combined with remote ischaemic postconditioning (RIPostC) improves the clinical outcomes of patients undergoing cardiac surgery. METHODS AND RESULTS From June 2009 to November 2010, 1280 patients who underwent elective cardiac surgery were randomized into the RIPC with RIPostC group or the control group...

Cardioprotective Effect of Extended Remote Ischemic Preconditioning in Patients Undergoing Coronary Artery Bypass Grafting: A Randomized Clinical Trial

Background: The cardioprotective effect of ischemic preconditioning has been known for many years. Since the temporary ischemia in the heart may cause lethal cardiac effects, the idea of creating ischemia in organs far from the heart such as limbs was raised as remote ischemic preconditioning (RIPC). We hypothesized that the extension of RIPC has more cardioprotective effect in patients undergo...

Long-term effects of remote ischaemic preconditioning in high risk patients undergoing cardiac surgery: follow-up of a randomised clinical trial

Results RIPC significantly reduced the occurrence of MAKE90 (17/120 (14.2%)) compared to the sham group (30/120 (25.0%); ARR, 10.8%, 95% CI 0.9%-20.8%, P = 0.034). In the 108/240 patients who developed post-surgery AKI (RIPC 45 (37.5%), Sham-RIPC 63 (52.5%)), 2 (5.3%) in the RIPC group and 11 (22.0%) in the Sham-RIPC did not recovery renal function by 90-days; ARR 16.7%, 95% CI, 3.2%-30.2%, P =...

Longer Length of Stay Increases 1-year Readmission Rate in Patients Undergoing Hip Fracture Surgery

Background: Proximal femur fractures are prevalent among the elderly and associated with substantial morbidity, mortality, and early readmission. Early readmission is gaining popularity as a measure of quality of hospital care and canlower reimbursement. A better understanding of the patient and treatment characteristics associated with readmissionmay help inform program improvement initiatives....
